This role is for practitioner(s) who pride themselves on providing a high-quality comprehensive triage for those identified as needing mental health support within a secondary mental health setting. The post holder is integral to accomplishing ease of access to mental health carefor Buckinghamshire’s population at primary and secondary care level in an efficient and timely manner. The successful applicant(s) will facilitate improved opportunities forjoint working with Buckinghamshire’s voluntary care sector, whilst contributing to the leadershipand development of personalised care , transformation agenda and Integrated care systems (ICS). As part of the Bucks Mental Health Gateway Multi-Disciplinary Team (MDT) your role will provide mental healthcomprehensive triage, signposting and professional adviceto patients , their family and your colleagues. You will work with professionals to facilitatebrief intervention for patients, to enable recovery within the community, whilst recognising the value in your skills to ensure prompt access to secondary care specialisms as appropriate. Successful candidate(s) on appointment as Gateway Mental Health Practitioner will receive formal bespoke induction, organised collaboratively between Oxford Health and industry experts by experience , with capacity forcontinued informal inductionat candidate’s consideration. You will be based within afit- for- purpose building on a day-to-day basis, with the opportunity for hybrid working. Candidates will be employed by Oxford Health and will receive clinical management andprofessionalsupervision through the trust, in addition to full training and development opportunities. The post holder will work as part of the Bucks Adult Mental Health Gateway team across the partnership and have specific responsibilities as a Gateway Mental Health Practitioner: To be main point of contact within Gateway. To work as part of the Gateway team performing comprehensive triage functions in line with Trusted Assessor Model. To provide specialist advise to referrer. To liaise with service users waiting to establish current presentation, give advice and formulate management / safety plans whilst waiting for specialist team’s assessment/intervention. To support the delivery of a high quality link worker provision into primary care, secondary mental health services, social care partners , community based resources and voluntary sector . This will include providing consultation and training to staff in a range of agencies in relation to the mental and emotional well-being of individuals experiencing severe / enduring mental health conditions. To support the Gateway/Team Operational Lead in the delivery of the service, ensuring it meets national guidance and local key performance indicators. To support the learning and development of new team members, students , Newly qualified, junior staff members when required Maintaining accurate and timely records. Ensuring legal requirements and subsequent amendments underpinning Children and Adult care are adhered to. To provide best evidence practice to service users which complies with legislation, policies and protocol and relevant quality, governance and local and national guidelines. Ensuring that the local demographics are taken into consideration for service provision and improvement. Risk assesses clients for suitability for interventions. To evaluate and make decisions on suitability of new referrals, adhering to the services referral protocols, and refer unsuitable clients on to the relevant service or provide recommendation to referrers following discussion with clinical lead.
