This is a 6-month Fixed Term Contract. Homebased. Today, over 4,000 staff provide social care services support to help more than 2,000 disabled people across the UK. In addition to offering high-quality care and community support, we lead campaigns to influence government to implement legislation that positively benefits the lives of disabled people. As well as fighting for disabled people’s rights, we work with partners to deliver support and life-changing programmes. Would you like to be part of the amazing work we do? We have a great opportunity for a Referrals Officer to support the Referrals Lead, ensuring our services achieve occupancy targets. You will identify and work with external parties to place people with a disability into our care homes with / without nursing or supported living accommodation, relevant to each person’s individual requirements. Your role will include actively supporting services with a key focus on high levels of customer satisfaction and occupancy, through an appropriate referrals network. With the ability to establish positive relationships internally and externally, you will develop action plans to effectively address occupancy shortfalls. Your other duties will include: Managing the occupancy system, recording, monitoring and reporting on all aspects of the referral process, including admissions and related queries. Focusing on improving the efficiency of the referrals process and financial performance. Ensuring the process is managed within agreed timescales and quality measures. Be the point of contact for key referral partners. About you…. Previous experience working within the social care sector is essential. Demonstrable experience in a similar referrals or placement management role. Direct experience of maintaining systems and reporting on data within a large multi-site organisation. Excellent communication skills, ability to build relationships and relay complex information. We offer 25 days holiday per year (pro rata).
