Role Overview This flexible 36-hour weekly role permits a combination of office and remote work. Office-based work is also an option, with free parking available at our Colindale office. Supporting the team manager in leading a supportive team, you will support staff to work with individuals to facilitate quick, safe hospital discharges and work post discharge to lead the life they aspire to live, placing their wellbeing at the heart of everything we do. As a leader in the service, you will be dynamic and support the team work in creative and innovative ways being comfortable and confident with change and in providing challenge when required. When required, you will conduct complex individual assessments and safeguarding enquiries using the appropriate tools and techniques. This will ensure clear, relevant, proportionate, and appropriate strategies are established to meet their identified outcomes. You will work in partnership with adults, their families, carers, advocates, health colleagues, and external organisations. Your goal is to provide appropriate, professional social work support for adults with care and support needs transitioning from hospital to community living. You may conduct Mental Capacity Act assessments in accordance with statutory guidance, ensuring adults are provided with the opportunity to demonstrate capacity where possible.
