Job description
The role We are seeking to recruit enthusiastic, motivated, experienced social workers to join the Preparation for Adulthood Team; a busy care management team working with young people with Special Educational Needs & Disability (SEND) as they make the transition from childhood to adulthood. Our vision in Warrington is for young people and vulnerable adults with SEND to have the best school or college experience, find meaningful occupation, live as independently as possible, feel valued and have greater control over the support they receive. You will need to be experienced in building and sustaining relationships with young people, vulnerable adults and their families. You will need to have excellent communication skills, be able to complete strengths based assessments, identifying needs, risks and outcomes with the individual. An excellent working knowledge of applicable legislation is essential as is the ability to work flexibly and effectively in partnership with a range of voluntary and statutory agencies as well as having good time management and organisational skills. A working knowledge of learning disability, physical disability, autism spectrum disorder, mental health and/or Court of Protection experience is essential.
